Management of family relationship informationfor a three-generation cohort study

Abstract

A system for inputting and storing family information, named \"BirThree Enrollment,\" was developed to promote a birth and three-generation cohort study (BirThree Cohort Study), and this system was operated successfully. In the study, it was necessary to satisfy many operational demands. Input information is overwritten and changed continuously. Complex kinship information must be quickly and accurately input and corrected, and information on those families not yet recruited must be retrieved. For these purposes, many devices are needed, from an input interface to the internal data structure. In the field of genetic statistics, a simple standard expressive form is used for describing family structure. This form has sufficient information for genetics; however, we developed this form further for our purposes in conducting the BirThree Cohort Study. To provide information about family roles as required in the BirThree Cohort Study, we expanded the data structure, and constructed the system that is able to be used for the daily operation. In our system, family pedigree information is stored along with initial clinical information, and enabled the input of all self-reported information to the data base. Operators are able to input this family information before the day is out. As a result, when recruitment is completed, family information will be completed concurrently. Therefore, it is possible to immediately know a certain persons family structure. By using our system, data correction was improved dramatically, and the system was operated successfully. This study is the first report of the method for storing three generations of family data.

A spatial analysis of childhood cancer and industrial air pollution in a metropolitan area of Colombia

BackgroundAir pollutants are considered carcinogenic to humans. In some European countries, an association with childhood cancer in children has been established. This phenomenon has not been addressed in Latin America, despite the spatial variability of air pollutants that may limit the extrapolation of the results to other geographical areas.\n\nObjectiveTo conduct a spatial analysis of the relationship between childhood cancer and air pollution from industrial sources in a metropolitan area of Colombia.\n\nMethodsIncident cases of childhood cancers were obtained from the Population-based Cancer Registry of the Bucaramanga Metropolitan Area (2000-2015). Local and focused cluster tests were used for the detection of spatial clusters and the Poisson multivariable model was used to evaluate the combined effects of spatial variables.\n\nResultsThe Kulldorffs focused test found a significant spatial cluster (p=0.001) around one industrial agglomerate and the multivariable model results suggests that the distance effect is modified by the directional effect of the wind.\n\nConclusionA spatial cluster of incident cases of childhood cancer occurred in the municipality of Bucaramanga. Our finding supports the hypothesis that childhood cancer might be related with industrial air pollution exposure in a Latin American city.

Chikungunya Outbreak in Bangladesh (2017): Clinical and hematological findings

A massive outbreak of Chikungunya occurred in Bangladesh during the period of April-September, 2017 and over two million people were at risk of getting infected by the virus. A prospective cohort of viremic patients was constituted and analyzed to define the clinical, hematological and long-term aspects of this outbreak. A 35-day long comprehensive survey was conducted in two major, neighboring cities, Dhaka and Mymensingh. One-hundred and eighty-seven clinically proven Chikungunya cases were enrolled in the cross-sectional cohort study. Additionally, a smaller group of 48 Chikungunya patients was monitored for post-infection effects for 12 months. Clinical data revealed that a combination of fever and arthralgia (oligoarthralgia and/or polyarthralgia) was the cardinal hallmark (97.9% of cases) of the infection. Hematological analysis showed that, irrespective of age groups, hemoglobin level significantly decreased and erythrocyte sedimentation rate remarkably increased in Chikungunya confirmed patients. However, the majority of the patients had a normal range of whole WBC and platelet counts; RBC counts for mid aged (40 - 60 years) and senior (61+ years) patients (especially in the females) were beyond the reference values. The post-infection study revealed that children had an early recovery from the infection compared to the adults. Moreover, post-infection weakness, successive relapse of arthralgic pain and memory problems were the most significant aftereffects, which had an impact on daily activities of patients. This study represents a comprehensive overview of clinical and epidemiological features of the 2017 outbreak of Chikungunya in Bangladesh as well as its chronic outcomes till the 12th month. It provides insights into the natural history of this disease which may help to improve management of the Chikungunya patients.\n\nAuthor summeryThe clinical profile, epidemiology and the economic impacts during the acute phase of Chikungunya infection has been studied quite rigorously. However, studies regarding the hematological features and chronic consequences are very limited. In this study, a dataset of 187 clinically proven chikungunya patients were analyzed for the clinical and hematological features at acute phase of the infection. Additionally, the long-term consequences till month 12 after the infection were studied for a smaller group of 48 patients. Clinical data revealed that a combination of fever and joint pain (arthralgia) was the cardinal hallmark in the acute phase of the infection. Hematological analysis showed that, hemoglobin levels of the patients were significantly reduced and erythrocyte sedimentation rate increased remarkably. Also, RBC counts for mid-aged and older patients were beyond the reference values. The post-infection consequence study unveiled that children recovered better from the infection compared to the adults. Further, post-infection weakness, successive relapse of joint pain and memory problems were the most significant aftereffects. Overall, the infection had moderate to severe impact on daily activities of the respondents. This study provides insights into the clinical and hematological aspects of Chikungunya infection during the acute phase as well as describes an account for its chronic outcomes which puts forward to the knowledge for clinicians and epidemiologists regarding the infection diversity and to help improved patient management.

Comparison of the predictive values of diagnostic tests subject to a case-control sampling with application to the diagnosis of Human African Trypanosomiasis

Case-control sampling to compare the accuracy of two binary diagnostic tests is frequent in clinical practice. This type of sampling consists of applying the two diagnostic tests to all of the individuals in a sample of those who have the disease and in another sample of those who do not have the disease. In this sampling, the sensitivities are compared from the case sample applying the McNemars test, and the specificities from the control sample. Other parameters of binary tests are the positive and negative predictive values. The predictive values of a diagnostic test represent the clinical accuracy of a binary diagnostic test when it is applied to the individuals in a population with a determined disease prevalence. This article studies the comparison of the predictive values of two diagnostic tests subject to a case-control sampling. A global hypothesis test, based on the chi-square distribution, is proposed to compare the predictive values simultaneously. The comparison of the predictive values is also studied individually. The hypothesis tests studied require knowledge of the disease prevalence. Simulation experiments were carried out to study the type I errors and the powers of the hypothesis tests proposed, as well as to study the effect of a misspecification of the prevalence on the asymptotic behavior of the hypothesis tests and on the estimators of the predictive values. The results obtained were applied to a real example on the diagnosis of the Human African Trypanosomiasis. The model proposed was extended to the situation in which there are more than two diagnostic tests.
