Integrating single-cell transcriptomics with Artificial Intelligence reveals pan-cancer biomarkers of brain metastasis

Brain metastasis (BrM) is a serious complication of advanced cancers and remains difficult to predict before clinical symptoms appear. To investigate shared transcriptional features of BrM across tumour types, we integrated single-cell RNA sequencing (scRNA-seq) data from malignant epithelial cells derived from six carcinoma types, including lung, breast, colorectal, renal, ovarian, and melanoma. We applied ScaiVision, a supervised representation learning method, to classify tumour samples based on BrM status. The models achieved high predictive accuracy (area under the ROC curve > 0.90) across all six cancer types. This analysis identified a consistent multi-cancer gene expression signature associated with BrM, defined at single-cell resolution. To evaluate the clinical relevance of this signature, we assessed its presence in tumour-educated platelets (TEPs) from blood samples of patients with and without BrM. The signature was detectable in platelet RNA and distinguished patients with BrM from those without, indicating that features of the BrM-associated expression program are reflected in blood-derived material. These findings demonstrate that a transcriptional signature of brain metastasis can be identified across multiple tumour types using scRNA-seq and neural network-based analysis. The detectability of this signature in TEPs supports its relevance in a non-invasive context and provides a basis for further investigation into its utility for BrM risk assessment.

Machine Learning Electroencephalography Biomarkers Predictive of Epworth Sleepiness Scale

Excessive daytime sleepiness (EDS) causes difficulty in concentrating and continuous fatigue during the day. In a clinical setting, the assessment and diagnosis of EDS relies mostly on subjective questionnaires and verbal reports, which compromises the effectiveness of available therapies. In this study, we used a computational pipeline for the automated, rapid, high-throughput and objective analysis of retrospective encephalography (EEG) data to develop objective, surrogate biomarkers for EDS, thereby defining the quantitative EEG changes in individuals with High Epworth Sleepiness Scale (ESS) (n=31), compared to a group of individuals with Low ESS (n=41) at Cleveland Clinic. Signal processing of EEG showed significantly different EEG features in the Low ESS group compared to High ESS, including power enhancement in the alpha and beta bands, and attenuation in the delta and theta bands. Moreover, machine learning algorithms trained on the binary classification of High vs Low ESS reached >80% accuracy. These results demonstrate that EEG data contain information in the form of rhythmic activity that could be leveraged for the quantitative assessment of EDS using machine learning.

Mass cytometry and artificial intelligence define CD169 as a specific marker of SARS-CoV2-induced acute respiratory distress syndrome

Acute respiratory distress syndrome (ARDS) is the main complication of COVID-19, requiring admission to Intensive Care Unit (ICU). Despite recent immune profiling of COVID-19 patients, to what extent COVID-19-associated ARDS specifically differs from other causes of ARDS remains unknown, To address this question, we built 3 cohorts of patients categorized in COVID-19negARDSpos, COVID-19posARDSpos, and COVID-19posARDSneg, and compared their immune landscape analyzed by high-dimensional mass cytometry on peripheral blood followed by artificial intelligence analysis. A cell signature associating S100A9/calprotectin-producing CD169pos monocytes, plasmablasts, and Th1 cells was specifically found in COVID-19posARDSpos, unlike COVID-19negARDSpos patients. Moreover, this signature was shared by COVID-19posARDSneg patients, suggesting severe COVID-19 patients, whatever they experienced or not ARDS, displayed similar immune dysfunctions. We also showed an increase in CD14posHLA-DRlow and CD14lowCD16pos monocytes correlated to the occurrence of adverse events during ICU stay. Our study demonstrates that COVID-19-associated ARDS display a specific immune profile, and might benefit from personalized therapy in addition to standard ARDS management. One Sentence SummaryCOVID-19-associated ARDS is biologically distinct from other causes of ARDS.

Ribosome recycling factor ABCE1 depletion inhibits nonsense-mediated mRNA decay by promoting stop codon readthrough

Nonsense-mediated mRNA decay (NMD) is an essential post-transcriptional surveillance pathway in vertebrates that appears to be mechanistically linked with translation termination. To gain more insight into this connection, we interfered with translation termination by depleting human cells of the ribosome recycling factor ABCE1, which resulted in an upregulation of many but not all endogenous NMD-sensitive mRNAs. Notably, the suppression of NMD on these mRNAs occurs at a step prior to their SMG6-mediated endonucleolytic cleavage. Ribosome profiling revealed that ABCE1 depletion results in ribosome stalling at stop codons and increased ribosome occupancy in 3 UTRs, indicative of enhanced stop codon readthrough or re-initiation. Using reporter genes, we further demonstrate that the absence of ABCE1 indeed increases the rate of readthrough, which would explain the observed NMD inhibition, since enhanced readthrough has been previously shown to render NMD-sensitive transcripts resistant to NMD by displacing NMD triggering factors like UPF1 and exon junction complexes (EJCs) from the 3 UTR. Collectively, our results show that improper ribosome disassembly interferes with proper NMD activation.
